In order to have your angel added to the Memorial Wall, just fill out the form below. It will email us with the information, and should also send a copy to you if you supply an email address.

We do allow up to 3 photos, but I don't know how to have it attach through the form yet. Those will have to be manually emailed to us. If you would like to attach a photo, choose Yes in the Photo option, and you will get the directions on how to send them to us on the confirmation.
